Job Description:
Registered Nurse RN, Personal Care Services
Territory - Lincoln County, St Charles County, and occasionally St Louis County (Brentwood Office); Up to a 50 mile radius
$2,500 Sign On Bonus | Part-Time | No Weekends + No Holidays

As a Registered Nurse, you'll contribute to our success in the following ways:
- Conduct health assessments to identify medical issues and determine appropriate plans of care
- Review, confirm, and update plans of care to ensure services meet consumer needs and regulatory requirements
- Provide supervision, orientation, and ongoing clinical oversight to homemakers, personal care aides, and home health aides
- Coordinate with Client Care Coordinators, clinical staff, and external agencies to ensure quality and continuity of care
- Perform start-of-care and routine supervisory visits in accordance with state and federal guidelines
- Monitor and report significant changes in consumers’ physical or mental status
- Educate caregivers and consumers using sound nursing principles and teaching strategies
- Ensure compliance with HIPAA, licensure, payer requirements, and agency policies
- Maintain accurate, timely clinical documentation and audit readiness
- Participate in staff meetings, in-services, and continuing education
- Support caregiver compliance, performance assessments, and quality improvement initiatives
- Represent Elara Caring professionally while fostering positive relationships with clients, caregivers, and partners
- Perform additional duties and projects as assigned
What is required?
- Graduate of an accredited School of Nursing (required)
- Minimum of one (1) year of nursing experience
- Home Health, Hospice or Home care experience preferred
- Active RN license in good standing
- Current CPR certification
- Strong clinical assessment and documentation skills
- Excellent communication, organizational, and time-management skills
- Ability to multitask effectively in a fast-paced environment
- Advanced proficiency in Microsoft Office (Word, Excel)
- Professional verbal and written communication skills
- Team-oriented, adaptable, and self-directed
- Ability to motivate and educate caregivers
- Reliable transportation with the ability to travel occasionally to support audits or operational needs
- Ability to sit, stand, bend, lift, and move intermittently
- Ability to lift up to 50–100 lbs as required
